Institutional Biosafety Committee (IBC) Review Schedule 2009
All submissions to the IBC must be made on one of the following forms:Recombinant DNA Registration Form
Transgenic Animal Registration Form
Human Gene Transfer Registration Form
Biological Toxin Registration Form.
IBC Registration Amendment Form

Submissions to the IBC will be reviewed following the schedule outlined below. Registration forms received on or before the submission date will be reviewed at the next IBC meeting. All registration forms submitted after the due date will be held to the following month for review. Approval/disapproval or requests for additional information will be sent to the Principal Investigator in writing following the IBC meeting.
Registration forms will be pre-screened prior to full IBC review. A lab audit is required as part of the review process for all work biosafety level 2 (BSL-2) or above. After the registration form has been submitted, the Biosafety Officer will contact the Principal Investigator to arrange a time for the lab audit. Biosafety training and animal biosafety training (if applicable) will be provided by EHS for all first time submissions at BSL-2 or above, or for previously approved protocols where overt exposure has occurred.
