Incident Reporting
INJURY OR ILLNESS
- Get Medical Attention
- Do not disturb the accident site until it has been investigated and released. Tools, equipment, materials or other evidence that might pertain to the cause of such accident may not be removed or destroyed until so authorized by the Labor Commission or one of its Compliance Officers.
- Do the paperwork (See below).
- Fill out the Workers Compensation first report of injury form 122 (also called an E-1)
- Fill out the EHS Incident Report Form.
- Determine if the accident must be reported to UOSH.
The University is required to notify UOSH within 8 hours of occurrence of all fatalities, disabling, significant and serious injuries or illnesses to workers. NOTE: EHS is the unit on campus authorized to make this notification. Tools, equipment, materials or other evidence that might pertain to the cause of such accident shall not be removed or destroyed until so authorized by the Labor Commission or one of its Compliance Officers. You are also required to investigate all worker injuries or occupational disease incidents.
Guidance on “disabling and serious” includes, but is not limited to the following: any injury or illness resulting in immediate admittance to the hospital, permanent or temporary impairment in which part of the body is made functionally useless or is substantially reduced in efficiency on or off the job which would usually require treatment by a medical doctor (examples of such injuries are any amputation, fracture, deep cuts, severe burns, electric shock, sight impairment, loss of consciousness, and concussions); illnesses that could shorten life or significantly reduce physical or mental efficiency by inhibiting the normal function of a part of the body (examples of such illnesses include cancer, silicosis, asbestosis, byssinosis, hearing impairment and visual impairment).
- If the accident is "serious" or involves a fatality, IMMEDIATELY notify EHS, 24 hours a day, 7 days a week. During work hours, call 581-6590. After hours, call 585-2677. EHS will notify Utah OSHA within the mandated 8-hour reporting time frame. This is the law.
- Fill out the Risk Management University of Utah Injury/Accident report form.
Return it to Risk Management.
- Fill out the EHS Incident Report Form.
