Prior to engaging in work with ionizing or non-ionizing radiation sources, all personnel must submit a Radiation Worker Profile application in the Safety Administrative Management (SAM) system. This profile application will provide prompts and links to the required training based on the information that the applicant supplies.
Students and individuals not affiliated with the University but want to use U of U equipment and need to take these trainings must become a University Affiliate (Type: 10054 – Bridge User). To do this, a University of Utah Affiliate Form must be completed and submitted by the department that is authorizing the affiliation for this work. Once Human Resources has approved this, the affiliate will then be eligible to complete the training.
Ionizing Equipment (e.g.: X-ray & Electron Microscopes (EM))
To receive and/or maintain an X-ray or EM device at the University of Utah will require an approved X permit, worker training, and regular inspections. Under no circumstances may an X-ray device be used at the U of U prior to inspection by EHS.
If you are interested in obtaining an X permit, please submit an application here: https://sam.ehs.utah.edu/ehsa/login?showassessment=RPR
Other guidance can be found through EHS document: RPR 30 – Analytical X-Ray Machines
Non-Ionizing Equipment (Laser, UV, EMF, IR, & RF)
The use of Class 3B and Class 4 lasers (regardless of the laser being imbedded) at the University of Utah requires an approved L permit, equipment registration, worker training, and may include regular inspections.
Other non-ionizing radiation equipment (e.g.: EMF, UV, IR, RF, magnets, and radar) at the University of Utah requires written standard operating procedures and appropriate training.
If you are interested in obtaining an L permit, please submit an application here: https://sam.ehs.utah.edu/ehsa/login?showassessment=RPR
Radiation Worker Application
If you are a PI that needs to become a Permit Holder for an Xray permit (X), Non-ionizing permit (N) or Laser permit (L), please submit an application here: https://sam.ehs.utah.edu/ehsa/login?showassessment=RPR
There is additional non-mandatory training available to guide the prospective Permit Holder for Laser and Non-Ionizing Permits:
