Radiation Work Permit (RWP)


The Radiation Work Permit (RWP), or radiation permit, serves as an administrative process for planning and controlling work with radiation sources based on the use. The radiation permit process requires the applicant to identify the type of hazard they intend to use and related information.

Things to Consider


The process of acquiring, possession, and use of ionizing and/or non-ionizing radiation sources is only allowed with a permit issued by the EHS Radiation Division. The radiation permit must have one Permit Holder and must be a U of U faculty or staff member with sufficient experience in using the applicable radioactive sources. Approval for these permits follows the application and review process, and the final approvals are made with various groups/personnel (e.g., Laser Safety Officer (LSO), Radiation Safety Officer (RSO), and the Radiation Safety Committee (RSC)), as indicated in the table below.

The types of radiation permits include:

Permit Type Applies to: Examples Approval Required
R Radioactive material (RAM) licensed, exempt, generally-licensed RSO* and RSC
X Ionizing-radiation producing equipment x-ray, electron microscopes RSO
L Lasers Class 3B/Class 4 (regardless if imbedded) LSO
AU Medical/Clinical Use Authorized User (AU) RSO and RSC
AMP Authorized Medical Physicist (AMP)
ANP Authorized Nuclear Pharmacist (ANP)
* The university’s RAM license must also allow for this usage. If it doesn’t, an amendment request to change the license can be made by the RSO to the Utah Division of Waste Management and Radiation Control (DWMRC). Allowance would then be based on the approval of the amended RAM license.

Radiation Safety Program Permit Application

To be added to a Radiation Safety Program permit, follow the steps below:

Before you begin:

  • Obtain the permit number from your lab. It will begin with R, X,
    L, or N, depending on the Radiation Safety Program
    permit type.
  • The application process will identify which trainings you must complete.
  1. Application Follow-Up and Edits
  • If a question is asked during the application process, you’ll receive an email from
    [email protected]. Click the “CLICK HERE” link in the email
    to return to your application.
  • If you need to update your application and no question has been asked, email
    [email protected] to request that your application be reopened for review.
  1. Accessing Required Systems
  • LearningHub AccessTo complete required training, you may need access to LearningHub by asking your PI or Manager
    to submit an affiliate form:

  • SAM (Safety Administrative Management System) AccessIf you cannot access SAM, request access by emailing [email protected] with
    the following:

    • Your full name
    • Your University Network ID (uNID – e.g. u1234567)
    • A brief explanation of why you need access
