A uniform signage program is in place at the University of Utah. This program is designed to communicate basic information about hazards which may be present within a given laboratory to non-laboratory personnel as well as emergency response personnel. If biological or radioactive materials are present within a space, additional signage may be required.
Signage is generated by the SAM system based on information gathered from the laboratory safety plan assessment.
It is important that the correct information be present on posted signs. Keeping the lab safety plan assessment in SAM up to date with the most current information for the lab is critical.

Critical Sections
PI – this should be the Primary PI responsible for the space. In areas where multiple PIs share the space multiple signs may be needed.
For Advice or Entry: This should be the name and phone number of an individual that can provide basic Lab information or can provide access to the lab to maintenance personnel or others that may need to enter the space during regular business hours. This may be an on-campus phone number and does not need to be monitored 24 hours/day.
Emergency Contact: This is the contact who would be notified if an emergency situation were to arise in the lab and would primarily be used by emergency response personnel. This information must be for an individual with knowledge of the space and with authority to make decisions regarding the space in an emergency – usually the PI or lab manager. The listed contact may not be administrative personnel that work for a department but do not work in the lab. The phone number may NOT be an on-campus number and must connect to a phone which is monitored 24hr/day, 7 days/week.
Signs are posted outside the lab in an acrylic sleeve which allows for easy changes of the signs when information needs to be updated. If a new sign or sleeve is needed contact your assigned EHS specialist and we will replace or install a new sleeve.
